To Do List
We provide an entire month to complete all of the necessary paperwork. Please complete it ASAP. DO NOT delay on the background check process!
- First, and foremost, make sure you sign up for Remind! This will be our number one form of communication this summer.
- Download the remind app and use code: @24mdcstaff to join
- This is how we will send out staff assignments - either Friday night or Saturday - and any other pertinent information.
- It is your responsibility to find out if you are working. We will expect to see you, if you are on the list.

- All employees must fill out a 2023 W-4 tax form. Do not use nicknames on this form, use your legal birth name. Please write your date of birth on the bottom of the sheet for our record.
- All employees must fill out a Direct Deposit Form in order to receive payment for working. If you are using a checking account, you must also attach a voided check.
- All employees must fill out a MEDICAL RELEASE/WAIVER and allergy action plan, if applicable. Type in your responses, print it, sign it (with a parent, if applicable), and hand it in with your paperwork. If you have an Allergy Action Plan, please return it with your paperwork, too.
- All employees must read through the Staff Manual, print and sign the last page, documenting that they have read and understand their job responsibilities.
- All employees must ensure they have followed applicable guidelines for performing a Background Check.
- If you were born after June 5, 2004, you must:
- have a Background Check completed by your local Police Department.
- fill out Working Papers. Working papers are available at your current school. If you are currently in 8th grade, you must complete your paperwork with the aid of Mt. View or St. Joseph's, etc. Only current 9th graders and up should go to their high school for paperwork completion. In hopes of easing the process, please see the step-by-step instructions. - If you born before June 4, 2005, you must complete a criminal-history Background Check (using Form D) AND a local Police Department Background check.
- For FORM D, please ensure that you fill out box 26 to say “Borough of Mendham – Recreation Department – 2 West Main Street, Mendham, NJ 07945”. This will prompt MorphoTrust to mail the results of the criminal check to the correct location.
Double check that on your Form D the Originating Agency Number (ORI #) NJPRR0000 is the same as the one you choose when scheduling. You SHOULD NOT choose NJ0141800 - Mendham Borough Police Department as the Originating Agency Number, which on the dropdown menu for the online scheduling option. You MUST scroll down to the very bottom choice of the dropdown menu to choose NJPRR0000 - Personal Record Request.

We WILL NOT accept paperwork at the training session scheduled for Sunday, June 2, 2024 at the Carriage House. We expect all paperwork returned to the Recreation Offices (6 West Main Street) by May 31, 2024. No exceptions will be made. Late or incomplete paperwork may jeopardize employment.
